I finally conceded that my GT220 must be one of the ones that doesn’t
work and bought a GT430 instead.
The GT430 works great using hdmi:CARD=NVidia,DEV=3 and I’m able to
stream the advanced codecs to my receiver. I updated the wiki with my
experiences.

I have sruggeled a bit on getting HDMI audio to work properly on two GT430 in two frontends.
I am using Alsa 1.0.23 and aplay -l reported four HDMI sound devices, 3,7,8 and 9.
In KDE/Phonon if I tested with HDMI only I got no sound. Using either of HDMI 3,7,8 or 9 worked with PCM sound.
In MythTV (0.24-fixes git version as of yesterday) the scan for devices only returned hdmi:CARD=Nvidia and no index 3,7,8 or 9. Selecting this device PCM sound worked, but DD sound from a recording only produced white noise.
I then remembered that when setting up the first card I read about setting a probe_mask for modprobe, but thought that I hadn't actually used that. After starting the working frontend I found that I indeed had used a probe_mask so that only one of the HDMI devices was visible.
Adding this to the second frontend got everything working with hdmi:CARD=Nvidia.
Stephen Warren of Nvidia has written a very descriptive document about HDMI audio.
This document was where I found the solution.
