I finally had the time to do this using these steps:
1.  force a scan of the missing channel using mythtvsetup, for my missing
channel, i knew it landing at qam256, chan 67.  After the scan, it tries to
insert the new channels, but fails, not sure why, maybe a bug.

It prints out this, but doesn't add the new channels:

3.  Found the freq in the dtv_multiplex mysql table, updated the mplexid to
a non-zero say 666 search for the freq '483000000'.
4.  Used the channel editor in mythtvsetup to add a new channel.  For
channel/freq, use 666.
5.  Changed the values of mplexid in the channel table of the new channel.
 In my case 666.
6.  Chan table, Changed the tvformat to ATSC.
7.  Changed the serviceid in the channel table row of the new channel to 2
in my case, see the printed out numbers
above: qam_256:483000000:KQED-HD::9-1:2 <--- serviceid

Hoping this helps the next person who has this issue.  This gave me have the
two HD channels I really wanted.
