I used to see it on my old system - though that was admittedly 
hampered by being a virtual machine on a not overly powerful host. I 
would regularly see corruption to recordings that would last only for 
the first minute or two before recovering. I also think it happened 
mid recording if another one started up, but obviously that's harder 
to pin down unless you make careful notes about what the system is 
doing. I put it down to disk activity, and in particular the "stuff" 
that happens each time a recording starts and ends.
If it turned off commflagging (which I did mostly), then it could 
just manage 2 recordings and one playback - but with the corruption I 
mentioned at the beginning of some recordings.

On my current system, dedicated box, two drives, two storage groups 
for recordings, but a raid 1 array on the same disks for OS and DB - 
I've yet to find it's limit.
