This means that myth attempted the recording, thought it was
recording, but there was an error of some sort and it did not record.

There are many reasons why that could happen, and a lot of those
reasons vary based on what your input source is.  What is it?  You may
also want to look at your mythbackend logs around the time that it was
attempting to record.  You may get some useful info there.
