You should be able to rig up a completely reliable direct channel
changing setup using some kind of IR blaster such as an IguanaIR or
equivalent but, instead of using IR emitters, instead use
optoisolators to couple an IR blaster output with a remote IR receiver
jack on an STB.  See the following thread, as well as other links
contained therein, or Google for something like "Optoisolator IR
blaster cable"
