Well, I get this sort of thing too, and I'm using an Intel HDA MB too - 
although not the same as yours.  You quoted aplay -L, so here's mine:

I just use the Analog stereo jack and at some time in the past needed to 
enable the Pulse volume control to get any sound at all.  On starting 
mythfrontend I typically get no sound until I have 'Configured' to use 
the initially unselected alternative from Analog Stereo  or 'Analog 
surround 5.1'.  (I see 13 options and 'off').  Once that's done either 
of these will work (ie make an intelligible noise) until, at random on 
leaving 'Play recordings', I lose sound and the log shows 'Pulse audio 
unable to connect'.  Restarting mythfrontend restarts the cycle.

It's infuriating, but it only infuriates me and I really haven't tried 
recently to get to the root of the problem.  So this is just 'me too'. 
Maybe I should get out a USB sound device I have somewhere.

All this is 0.23-fixes, Fedora 12, 64 bit.
