The 'sideways' PCI card style cases from the major suppliers are well
thought out. I have a SilverstoneTek LC-11M which does this.
The SilverstoneTek LC19 which was linked to earlier in this thread comes
with a both solid and flexible risers.

The dual riser card supports the finger edge of the card, and the other
backplane is clamped to the back of the case at the 'top' and 'bottom'
of the card. Extremely solid where the motherboard PCI slot alignment is
correct as with my Asus motherboard.
The Via SP13000 motherboard I used to use was not quite properly
aligned, so I used a flexible extender between the mb and the riser. I
had no problems with it. The front end of the card might be slightly
less solidly supported but not enough to matter. And in my case, the
front end was supported by the mess of wires and cables which were
stuffed under the front end!

I changed motherboards only because the Via would not do highdef.

I have since changed cases only because the power supply died and was
not readily replaceable.
