Looks like your theme install is broken--missing setup.xml and, 
possibly, some other parts.

There's no way the log output from above could be from a MythTV 0.21 
install.  Those messages are definitely from MythTV 0.22 or higher.  Do 
you, perhaps, have multiple different-versioned MythTV installs on the 
same system?
