... and the PVR cards are not V4L cards creating nuv files, AFAICR.

First thing I would do is delete all capture cards and recreate them
(ensuring they are configured as hardware MPEG2 cards) in
mythtv-setup. Also make sure the relevant /dev/videoX nodes are being
correctly created, and that the ivtv driver is working normally
outside of MythTV.
