I'll lay odds that ALSA:pulse is causing it.  When you change that to 
use ALSA:default (and do not redefine default to use Pulse), it works, 
right?  Myth doesn't support using PulseAudio, yet (as Pulse doesn't 
provide the necessary functionality, yet).
