Sometimes they don't even try. I saw pictures recently of a Comcast
install where they couldn't dig up the frozen ground to install a new
drop, so they simply ran the wire through the gutter, then across a
driveway and a lawn.
When it comes to power utilities, another issue is that underground cables
need to be larger for the same current rating than overhead wires. This
is because the overhead wires are cooled by air circulation while the
underground wires aren't.
