Just at first glance it looks to me like:

1.  Your audio configuration in Myth isn't set right
2.  Your video output isn't either (video card drivers)

I haven't been following the rest of the thread but you're unlikely to get
good results until those are fixed.  Note the "Check mixer" line as well as
"Could not find suitable Xvideo surface".
