I did three things:

1) I ran 'apt-get update mythtv-themes', and;
2) I stopped using some older, non-compatible themes (particularly
MythTVCenterOSD)
3) Went into MySQL manually (under the settings) table and deleted the
skin value for my xbox frontend.

As for writing it up into a wiki; it's possible, although I went
through so many trial and error steps to get it working... it might
not work 100%.  I think following the sequence of messages in this
thread is the best way.
