Not really a patch, but a configuration issue:
The following Device section enables full HD output with a Intel 945G 
chipset:

Section "Device"
         Identifier      "Videocard0"
         Driver          "i810"
         Option          "XvMCSurfaces" "7"
         Option          "CacheLines"  "2048"
         Option          "LinearAlloc" "16384"
EndSection

I'm using an LG 37LB4D TV and the output is awesome.
Thanks for the pointer to that page.

The only oddity I have remaining, is that the OSD displays *behind* the 
recording...
