The card is working now. I threw it into a windows box and it
immediately caused bsods that seemed to be irq related. I noticed on
the windows box that it was trying to use irq 9 along w/ almost every
other pci card in the system. In that box's bios I can force a given
pci slot to use a particular irq. Once I forced the slot it was in to
use irq 11 the bsod stopped and the card worked.
On my mythdora test system I can't manually assign irq's to particular
pci slots in the bios so I just swapped pci slots and the card was
recognized properly.
For future reference, here is the relevant part of the dmesg when it
loads properly:
