What rpms have you installed I don't think you have the correct firmware
installed.
on my Fedora 5/6 system I ran this to install my PVR-350 ivtv software.
yum -y install ivtv-firmware
yum -y install ivtv-kmdl-`uname -r`.`uname -m`
yum -y install ivtvdev
your /etc/modprobe.conf looks fine to me (its the same as my working one
is) but for the PVR-350 Output to work you do need to add this to the
modprobe.conf file also.
alias char-major-81 videodev
did you add “vga=791” to the end of all the vmlinuz lines in /etc/grub.conf?
