Never hurts to run it and check the values for sanity and modify the 
newly added values to your preferences (instead of defaults).  However, 
database upgrades are (and have been) done automatically by 
mythtv-setup, mythbackend, /and/ mythfrontend, so it's not a "mandatory" 
step.
