A quick guess on your particular error, is that you are still running 
the daemon, when you attempt to test the hardware with irw.

Do 'service lirc stop' (or equivalent), then test the setup with mode2, 
irw and irrecord.
You may have to change the permissions for the /dev/lirc socket, in 
/etc/udev/rules.d/10-lirc.rules  (or equivalent), to allow the mythtv 
user access....chmod 666 /dev/lirc* is a good check-step.

Note that the NAMES of the buttons must match in lircd.conf and lircrc. 
  The button NAMES are what get passed between the various portions of 
the software.
