Your problem is almost definitely in Myth, not IvyTV.  However, if the 
last lines of the log provided occurred when Myth attempted to set video 
parameters, you're not using 720x480.  Note that something is setting it 
to 480x480.

Although the resolution is not necessarily the problem, the fact that 
it's not the resolution you think it should be indicates that you're not 
properly setting your recording parameters.  Also, 720x480 is the 
resolution that's least likely to cause problems, so it's the best one 
to start with.

Make sure you select the Next/Finish buttons and don't "Esc" out of the 
page.
