Sorry, I have been away.  To answer all your questions including Scott and Intense Red:

No, the card does not stop responding - it just shows nothing at all on the screen starting from bootup.  I put in a different PVR-350 and it works fine.  So, that includes doing a full cold reboot.
 
I will check out and try the various troubleshooting tips.

  When does the PVR-350 stops responding. Maybe when you are changing channels in Live TV?
  The ivtv driver has a bug in the 0.4.5 and in the 0.6.2 version when changing channels in Live tv. This is fixed in the 0.6.3 and in the 0.4.6 cvs release
