Did you load the lirc module?

Did you start lircd?

If you ensure the module is loaded then re-run lircd, does irw just
silently return the first time you run it, then give the "connection
refused" when run after that?  If so you are running into the
/dev/lirc0 instead of /dev/lirc problem.  Search the archives for
/lirc "connection refused" lirc0/.  Depending on your linux
distribution you may be better off creating a link or modifying the
lircd line in one of several config files.
