I picked it up on ebay for something like $10+$10 shipping.  It is the " 
Impact VCB  "

You must have BOTH:
RCA Audio cables OUT of the Dish box and IN to the PC
SVideo cable OUT of the dish box and IN to the PC

The RCA cables are for Audio, the SVideo cable is for video...you can use a 
composite cable instead, but you will have less picture quality.
