Mike,

Is your PVR-250 connected to the cable box through the composite or 
S-Video input?

If I recall correctly, /usr/bin/ivtvctl -p 4 works for the coax input of 
the card. Since you have nothing connected to the coax in, the snow is 
normal. Try -p 5 (/usr/bin/ivtvctl -p  5) for a composite (RCA) input or 
-p 6 (/usr/bin/ivtvctl -p  5) for the S-Video.
