I have the same issue.  I gave up on zsnes and switched to snes9x it
detects the gamepads correctly everytime, Although I still havent
figured out how to start it in full screen mode yet.

I think it's due to this but I don't know how to fix it.  I used all the same switches in my command prompt run so it should be similar.  I thought maybe it was joystick navigation in myth conflicting with mythgame so I tried recompiling with the --disable-joystick-menu but it didn't help.  Could mythfrontend/mythgame be affecting how the joystick is detected in zSNES??
