Digital cable put you in the same situation as people who have
satellite dishes or other tuner-required setups.  To get all of your
channels, you have to run through a different box first.

For your setup, you're looking to do something like this:
Wall -> Digital Cable Box -> MythTv (PVR-350)

Then the output of your PVR-350 can be connected to the TV.  This
gives you all of the benefits of mythtv (timeshifting, livetv, etc).

The Digital Cable Box will have to be controlled by the MythTv PC. 
This means the analog tuner in your PVR-350 doesn't do anything (I'd
suggest using the S-Video connection actually).  This is usually
accomplished with some sort of serial cable or IR blaster that gives
channel change commands to the digital cable box.

All of this means that you lose your standard TV functionality of
channel surfing and directly interacting with the digital cable box
(via it's remote control).  Instead you tell mythtv to do things for
you (it in turn tells the digital cable box to do things).
