The ivtv module isn't loading properly. Try posting the results of: $rpm
-qa ivtv
$rpm -qa kernel-module-ivtv\*
$cat /etc/modules.conf
You should have the following line in /etc/modules.conf:
alias char-major-81-0 ivtv
