I bought one from Hauppauge lately as well, and it
works fine as an encoder (I don't use it for TV-Out
yet).

It came in an orange box (using the same box design as
the newer PVR-250's), and on the box, it says, "model
990".

Here's what lspci -v -v says:

(Note: I put in the info for both my PVR-250 and my
PVR-350. It seems that I have a Rev. 1 PVR-250 which
has an iTVC15 chip, so I can't identify which card is
which, since I don't know the model numbers off-hand).
