Aha, I believe you *have* followed things to a T. But the nvidia
driver's auto-detection of connected displays doesn't like a lot of AV
switches. Many of them don't support the flow of two-way data to allow
proper detection. Try adding the line:
Option "ConnectedMonitor" "TV"
Or, for mirrored display on monitor and TV, try:
Option "ConnectedMonitor" "CRT,TV"
(Those are from memory, but I think they're right -- double-check
nVidia's docs to be sure).
