Yes I did run setup again, both after installing PVR-250 under .12 and again
after upgrading to .19. I wiped out the card and channel info both times
because I was fooling around with inputs, regular cable, digital cable
etc...

Did you run setup again when you upgraded to 0.13?  If I remember 
correctly, I had to do that when I upgraded because there was some new 
functionality added and it doesn't get configured properly until you run 
  setup (again).
