Looks like you forgot the "v4l" module in /etc/X11/XF86Config. Just
make sure the Section "Module" contains a Load "v4l". See man v4l.

xv is the best method for nvidia cards, but mplayer can do a lot
more. Try mplayer -vo help to check the list.
