Chris,

It looks like you hit the nail right on the head.  I have changed my 
DisplaySize setting to the following:

        DisplaySize 138 122 # yeilds approx 133x100dpi @ 720x480 (4:3)

My display is once again full-screen and the aspect ratio looks good!
