Your tuner type is set wrong. You can change it when you load the ivtv 
module. Add

options tuner type=#

for older ivtv drivers or

options ivtv tuner=#

for newer drivers. I'm using 39 for my 350, which isn't totally right 
but it works for the channels that I watch.
