I just built a Myth box a few weeks back, and I spent a bit of
effort trying to keep the thing quiet: 

 * PVR-350 (so I don't need much CPU throughput).

 * 1GHz VIA C3 processor with fanless heatsink.

 * Samsung SpinPoint 120GB drive known to be very quiet.

 * Antec Overture black case (17" 'stereo equipment' width)
   designed to be quite with temperature contolled fan.

I'm pretty happy with the results.  Even with the cover off,
the only time I can hear the thing at all is when I'm building
a kernel or something and the CPU warms up causing the fan to
rev up a little.  It's very quite when just recording/watching 
TV: I can hear my laptop running in the other room, but I can't
hear my myth box 8 feet away.

That said, the cheap silver CD drive I put in is horrible. It
sounds like a fully loaded Boeing 707 spinning up ready for
takeoff. Fortunately I only use it when I need to boot from CD.
