If ntp discovers that your clock is too far out of sync, it will step
the time instead of adjust it gradually.  That seems to be what's
happening on your system.  This can be for a number of reasons:

1) your system clock is so bad that it constantly runs too fast or
   slow: you could try to correct this with tickadj
2) your system clock goes screwy, maybe because of APM: try disabling
   APM

Probably the best way to deal with it would be to run ntpd with the
'-x' parameter, which tells it to always slew the clock and never step
it.
