Set the card as a "Hardware MPEG Encoder card" in setup, choose 
"Hardware MJPEG" for the codec. I would have thought that would be 
pretty obvious...

Just out of curiosity, how carefully have you read the documentation 
that's out there. Tons of what you're asking has already been covered 
in documentation, and/or the mailing lists.

I believe it uses the default, unless you've specified a profile for 
the recording to use.
