Sorry, didn't have time to check my log file last night (was working in Windows all night), but here are some answers to your other questions:
Nvidia driver is 1.0-4363 (I didn't even know there was a newer release until today)
XFree is I *believe* 4.3 - I am running Red Hat 9 and keep all my packages updated with apt-get. It is definitely 4.1 or above, but I am 99% sure it's 4.3.
My config options are exactly the same as yours except I do not specify the TV standard and my overscan is set to 0.7. I would post my file itself but I am not at home (and am going on vacation tonight, unfortunately, so probably won't be able to until next week).
My TV is my only output - I don't use Twinview or anything, and have only one monitor defined. I think I remember seeing you had two - maybe try only running to your TV and see if that does anything (seems like a longshot, but maybe it's trying to overscan on the wrong screen).
Beyond that I'm not sure what else I can say right now - if you can wait a few days I'll still try and post up my log and config files.
