Your problem probably does not have anything to do with Alsa or sound output 
since everything else seem to work. Are you using the latest ivtv drivers 
from CVS ?

I had the same problem recently after upgrading from MythTV 0.10 to CVS 
version. A reboot after upgrading to the latest ivtv driver solved the 
problem.
