[mythtv] FFmpeg 4.4.1 problems

I have cleaned up the patches as well as I could.  Each now compiles on 
top off the previous commit.

No new mythtv changes.

The patches should be ready now.

As an aside, I think the ffmpeg code would be much cleaner and clearer 
as C++ instead of C.
