Do you happen to have an external channel changer specified?
Your issue sounds pretty similar to problems I had and your backend logs 
are the same and that was the cause of my problems.
Change to using myth system events if it is the case. Happy to share a 
script if you'd like, though it would need some tweaking for your 
specific requirements.
