OK.  I assumed you checked for additional versions of mainmenu.xml and
that you restarted.

Can you run mythfrontend from a terminal and post the output?  What
you will need to do is start mythfrontend, and navigate to the menu
page where your plugin should be located and look for relevant output
that indicates what is going on.
