Notice the comment at the top of the optimize_mythdb.pl script that 
suggests running the script in a daily cron job (ideally at a time when 
recordings are least likely to occur)?  :)

Yep (perhaps you did notice the comment).

That would be up to packagers.

Eventually, Myth will do all required database maintenance automatically 
by itself.  I have partially-written patches to allow this, but it 
requires a lot of changes to make doing so safe (from a "without the 
safety changes, automatically repairing the DB could destroy the entire 
DB/all its data" perspective), therefore it will be a while, still.
