[mythtv] Ticket #4503: Guide shows duplicate channels after multirec if channum's are the same

Daniel, I don't believe I'm explaining myself well enough with this one. Why
would the preferred behavior be to have duplicate channels listed in the
guide one of them hiding a channel on a different source with a different
callsign that just happens to be unavailable? This one very much appears to
be a bug and I think people with similar setups will view it the same way.
There is some confusion on how Myth detects duplicate channels. I've heard
conflicting answers. Is it:
1. Callsign OR channum
2. Callsign AND channum
Right now the behavior in this bit of code adheres to answer 1 as well as
the new duplicate detection in fillChannelInfos (if I remember correctly). I
always thought the answer was 2 given the possibility of having different
sources with channels that have the same channum.
