Please open a ticket and post this list there, then we won't forget. It will 
help if you can test these streams with the svn version of ffplay (ffmpeg). 
Both xine and mplayer also use ffmpeg, so in theory there is nothing that 
they can play than mythtv can't with a little work.

Yes, in current trunk although it's not enabled by default at the moment. Ask 
Janne as I don't know the details.

If may have an effect, but probably not. If you try this and it makes a 
difference then mention it in any ticket, it will help narrow down the cause 
of the problems.
