[mythtv] Menu problems with internal DVD player in ATrpms build 0.21-178_trunk_r16254

Just updated 0.21 fixes to hopeful resolve the item selection frame not
showing. I have a mytharchive dvd so I can reproduce the disabling of audio
in dvd menu on mytharchive dvds.
I'll work on that soon.
