[mythtv] Ticket #5486: mythvideo cast order is backwards

Anduin, so far mysql seems retrieve the cast in the same order as it was inserted. Could you please clarify "There is almost no way for this to order properly over time", as I can't say I have extensive experience with mysql other than mythtv.

If you think that it will eventually will go out of order, I can do a patch to add an order field to the DB.

Also, would it be ok if I generate a patch to remove "None defined" for cast in videoutils.cpp and replace it by ""?  In my personal theme I show the cast list on the screen.  For those movies that have no cast list available from imdb (or other source), then I prefer to see blank rather than "None defined".  Or alternately, I could generate the patch to show nothing on the screen if it is set to "None defined", that way "None defined" should still be shown in the pop-up as it should be.
