Under fedora that error would be fixed by a 'yum install QtMysql'  (or qtMySql). That is, I think the only driver in fedora whose name is case sensitive and I cannot remember the precise spelling. I have  no idea what the equivalent windows dll is called, but you clearly do not have it loaded.
As to the other error, do you have your hostname (silver??) tied to the IP address? In linux this is in the hosts file. I think there is a windows equivalent..
