[mythtv] Ticket #4270: MythTV on Windows (with MinGW)

 I have started on documenting my attempt to follow in Andrei's footsteps
and build the MinGW verson of myth from SVN.  It's early days, I know, and
the patches proabaly aren't even into SVN yet, but I figured the sooner it's
documented, the better.
