[mythtv] MythVideo internal player filetype case sensitive

Is something holding this back? I thought I saw a patch for this last  
spring but it didn't show up in .20.

I find the internal player is hopeless for 2 out of 3 dvd menus so it  
would be good to have "play with..." in the popup menu.
