[mythtv] Key binding: local vs. global context

I had the same desire as John did so am glad to see the patch, and
believe that this is a good change to make. Those who want the global
binding to kick in in a particular context can always just make sure
the local binding is empty or changed to something else, but right now
there's no way to do the opposite.
