[mythtv] MythDVD another Internal failure mode?

OK, debug patch applied and I've run through all output options that I'm
given - these being XvMCVia, Standard XvMC, ffmpeg and libmpeg2. Only the
last one displays the language selection menu from the DVD, all others
exit to the MythDVD menu.

I hope is provides some useful feedback
